How contact information is handled in this United Dental Specialists demonstration.
Scope of this demonstration
This temporary version lets you review the design and test the United Dental Specialists form. Messages are sent only to the authorized reviewer, yulio.ortiz@gmail.com. This demonstration does not confirm appointments, process payments, or send SMS.
Form information
The form requests your name, email, phone number, message and consent. Do not include records, diagnoses, clinical images, or other sensitive medical information. Data is transmitted using HTTPS and email through Mailgun to the test recipient; it is not stored in a website database.
Cookies and security
A necessary session cookie protects the form. The token expires after two hours. Spam controls retain a hashed IP identifier for one hour without logging message content. The hosting provider may retain technical logs under its own policy. No advertising trackers or analytics have been added.
Comments and optional saved details
Blog comments are sent to the reviewer for moderation and are not published automatically. Only if you select the remember option, your browser stores your name, email and website for 30 days. Comment text is never saved in that reminder. Clear the option to remove those details from this browser.
Retention and requests
Received email remains in the reviewer’s inbox until manually deleted. To request deletion of a test message, reach the reviewer through this demonstration’s contact form. Recipients, retention and this policy must be reviewed with the practice owner before the final launch.
Third-party links
The patient portal, Google Maps and social networks are external services with their own terms. They open only when you choose their links.
